How to Register
- Simply download the Repeat Prescription form below, complete and return it to us by post, email or in person.
It is important that we receive a fully completed repeat prescription form containing the correct data otherwise a Repeat Prescription cannot be issued.
No Need to contact your GP
We'll contact your doctor so there’s no need to make a GP appointment - we can order and collect your repeat prescription from your surgery. When your Doctor needs to see you or review your medicines your Doctor will let us know and we will inform you.
Our Repeat Prescription service is Confidential
Once you've completed your form we can store your repeat prescription information, this is completely confidential and is only used by our pharmacist to issue your prescription.
Collection & Delivery
Your medicines can be collected in store from Whitley Bay Pharmacy or delivered free to your chosen address.
Please note that Professional Pharmacy guidance rules requires someone over the age of 18 to sign for any medicines delivered.
